This is not our HIPAA notice
Health care providers give patients a separate document called a Notice of Privacy Practices. That notice covers your medical records and the protected health information we handle as your hospice provider, and it is governed by HIPAA.
This page is not that notice. It covers only this website. If you are a patient or family member and you want our Notice of Privacy Practices, please call us at (949) 426-1693 and we will provide it.
What we collect when you use this site
Information you give us
Our contact form asks for your name and phone number, and optionally your email address and a short message. We use this only to get back to you about your enquiry.
Please don't send medical details through the form. A website form is not a secure way to share health information. Tell us just enough to reach you — a name and a number — and we'll call you back and talk it through properly. Day or night, a person answers: (949) 426-1693.
Information collected automatically
We use Google Analytics to understand how people use this website — which pages get visited, what kind of device people are on, and roughly which city they're in. It tells us whether the site is working. It does not tell us who you are, and we never use it to identify or build a profile of an individual visitor. Google Analytics does not store your IP address. Our web host also keeps standard server records of page requests, the kind every web server keeps.
The services this website uses
We use a small number of outside services to run this site. Each one is listed here so you know who handles what.
Google Analytics
Measures how the website is used — pages viewed, device type, and general location. It never receives anything you type into the contact form.
Web3Forms
Delivers our contact form to us by email. It receives what you type into the form.
Google Fonts
Supplies the typefaces used on the site. Loading a font shares your device's IP address with Google.
GitHub Pages
Hosts the website files and keeps standard server logs.
Cookies
This website sets a small number of cookies for Google Analytics. They count visits and tell us whether someone is returning to the site. We do not run ads on this site, and we do not use cookies for advertising or to follow you to other websites. You can block or delete cookies in your browser settings at any time, and every page here will still work.
How long we keep information
Messages sent through the contact form are kept only as long as we need them to respond to you and follow up.
Analytics information is kept for 14 months, after which Google deletes it automatically.
What we don't do
- We do not sell your information.
- We do not share it with advertisers.
- We do not use it to target you with ads anywhere else.
- We do not send marketing email to people who fill in the contact form.
Your choices
You can browse this entire site without giving us any personal information — every page works without filling in a form.
If you would rather not be counted in our analytics at all, you can install Google's official opt-out add-on at tools.google.com/dlpage/gaoptout, or simply browse in your browser's private window.
If you have contacted us through the form and you would like us to delete your message, call (949) 426-1693 or write to the address below, and we will remove it.
Children
This website is intended for adults making care decisions for themselves or their families. It is not directed at children, and we do not knowingly collect information from children.
Changes to this notice
If we change how this website handles information, we will update this page and change the date at the top.
